Documents needed for this form

You will need to upload digital copies of the following forms for your application, so please have them ready:

  • A colour photo or sketch of your stall or vehicle
  • A plan or map of your precise trading location (if not mobile)
  • Any certificates or evidence of attending relevant food handling/hygiene training
  • Proof of identity and address, such as passport or DVLA photo card driving licence
  • A copy of your certificate of insurance (original) covering third party and public liability risks
  • Confirmation or copy of a commercial waste agreement
  • Copy of your gas safety certificate issued by a GAS SAFE engineer
  • Copy of your electrical safety certificate issued by a NICEC registered contractor
  • Confirmation that the trading unit is registered under the Food Premises (registration) Regulations 1991
  • Copy of your application sent to residents within 100 metres of the proposed trading site
  • Basic DBS check certificate - You can get one via this link. Get a DBS certificate.

Right to work in the UK

If you need to prove your right to work in the UK, then you will be able to generate a 9 digit share code to submit with this application.
