When completing this form, you will be asked for the following:-

Passport
DVLA license
Proof of address
Proof of National Insurance
An emailed passport type photograph of yourself (jpeg)

A Safeguarding training certificate less than 3 years old, if already completed

There is a 25MB file size limit for uploads. Photos taken directly on a smartphone are usually too large and may not be accepted. This form does not accept photo file types e.g. JPG, PNG.

To ensure your documents upload successfully, we recommend using your smartphone’s built‑in document scanner (usually found in the camera app) or a scanning app. Save the files as PDFs or similar document formats, which keeps the file size small and ensures your application is submitted without any issues.

Before a licence can be issued the following will need to have been supplied to us or passed:-

A Safeguarding training certificate less than 3 years old, if not already completed and supplied with your application.
Your DBS certificate
Pass our Knowledge Test
Be assessed as being “Fit and Proper”.

Licence applications must be completed within 8months of applying.
